CARLIN, Nev. (KRNV & MyNews4.com) -- United States Department of Labor is reporting a man died in a Carlin mine after falling through a hole.
On August 31, 2012, Allen Campbell, 49, was killed when he fell through the hole that had developed beneath bridged material in an open stope.
The U.S. Dept. of Labor says the accident occurred due to management's failure to ensure that the victim used fall protection when working near an open stope where there was a danger of falling.
