RENO, Nev. (MyNews4.com & KRNV) -- A foster home is under scrutiny after an audit directly claimed that it did not provide reasonable assurance it protects the safety of youths in its care. The state audit said medical protocol inside the home needs improvement.

Etxea Services LLC runs two foster homes in Reno, one for boys and one for girls. Last month's audit investigated the company's medical policies, filings and medication distributions.

Documents obtained by News 4 show Etxea Services did not follow efficient protocal from July 2011 through September 2013. According to the report, staff failed to follow company policies when administering medications.

Files for two residents had no medical recordings for one month. Physician's orders and pharmacy instructions were missing from up to four files, as well. Also, one staff member failed to ask a resident to show his empty cup or check for pills on his tongue after taking medication.

Etxea officials did not want to speak on camera Thursday, but Director Bertrand Ithurraide released the following statement:

"The changes are from the old way we used to do meds to the new regulation now. We overhauled our old med system to meet the new state and legislative requirements."

The legislative audit calls for updates to the center's medicine policy.
